Rosie O’Donnell has shared insights into her recent facelift, showcasing before-and-after photos that illustrate the subtle changes resulting from her procedure. This revelation comes from her Substack essay and an Instagram post published earlier this week, where she discussed both the emotional and physical aspects of her decision.

The 64-year-old actor and comedian disclosed that she underwent a lower deep plane facelift in January. She described the procedure as one that lifts deeper tissue beneath the facial muscles, avoiding excessive skin tightening that can lead to an unnatural appearance.

In her post, she shared the photos with a straightforward caption: “THE B4 & AFTER — story on substack.”

In her essay, O’Donnell expressed that she never anticipated having cosmetic surgery. She previously held strong beliefs against facelifts, viewing them as a moral issue tied to aging and feminism.

She wrote, “I used to feel very strongly about facelifts. Not casually — morally. I had assigned myself as head of all women who would never — ever.”

Her perspective began to evolve after she lost approximately 50 pounds while taking the prescription weight-loss medication Mounjaro. She noted that the weight loss altered her appearance in unexpected ways, particularly making her wrinkles more pronounced and giving her a fatigued look.

O’Donnell explained that she initially attempted to embrace the changes positively. She reassured herself that aging naturally was something to celebrate. However, she soon found herself questioning that mindset, pondering, “Umm how earned does it have to look?”

She also mentioned that her child, Clay, was strongly against the idea of surgery. Clay reminded her that many young people look up to her and would be influenced by her choices, stating, “Young women look up to you,” and added, “I wouldn’t be able to respect you if you did it.”

These remarks prompted O’Donnell to reconsider the procedure for a time, forcing her to reflect on her beliefs and whether she was being inconsistent with her previously held values.

After careful consideration, she decided to make a choice that felt right for her. She found a surgeon she trusted, recommended by friends who had undergone similar procedures and still appeared natural.

Post-surgery, O’Donnell described the outcome as subtle and natural. She feels she now looks like a more rested version of herself rather than a completely different person. She humorously noted that, surprisingly, no one around her seemed to notice the change.

She wrote, “Not one person in my life has noticed. I went through a full existential feminist crisis, had my face and neck surgically altered, and the result is… zippo.”

Despite the lack of reactions from others, she found peace in the experience. She shared that she no longer feels like she is in constant conflict with her reflection, stating, “I didn’t disappear, I didn’t become someone else — I just stopped arguing with the mirror. And maybe that’s enough.”

O’Donnell’s reflections highlight the personal and complex nature of decisions regarding appearance, especially when intertwined with public expectations, aging, and identity.
